GENERAL PRACTICE QUESTIONS
Integrative Body & Mind is a direct primary care and wellness practice located in Fort Morgan, Colorado. We focus on whole-body health by combining preventive primary care, hormone therapy, and aesthetic medicine in a comfortable, patient-centered setting.
Our office is located at 931 Lincoln Street in Fort Morgan, Colorado. Convenient parking is available on-site.
We see patients Tuesday through Friday by appointment. Hours may vary depending on provider availability. Give us a call at 970-436-4371 to set up a time!
All medical care is provided by Sarah Whitney, FNP-C, PMHNP-BC, founder of Integrative Body & Mind.
We care for individuals and families of all ages, from newborns to older adults, and offer both general wellness and specialty services such as hormone therapy and aesthetics.
Yes! We are currently accepting new members for Direct Primary Care, as well as patients for hormone and aesthetic services.

Yes, follow-up visits and medication reviews can be done virtually for your convenience.
We reserve same- or next-day appointments for our DPC members whenever possible.
Bring a photo ID, a list of current medications and supplements, and any recent lab results or medical records that may help us personalize your care.
MEMBERSHIP & BILLING
DPC is a membership-based model that gives you unlimited access to your provider for a simple monthly fee—no copays, no hidden charges, and no insurance hassles.
Members pay a flat monthly fee that covers primary care visits, same- or next-day access, and ongoing communication with your provider. You can message, call, or schedule appointments whenever you need care.
Most routine visits, annual physicals, chronic condition management, and basic in-office procedures are included. Rest assured, we will always discuss all available options when items that are not covered are needed.
Membership rates vary by age and service level. Please visit our DPC page for current pricing details.
We do not bill insurance. By removing the insurance middleman, we keep costs low and care personal. Many members keep a high-deductible insurance plan for emergencies or specialty care.
Yes. You can use insurance for any outside services, such as imaging or specialist visits.
Yes, many Health Savings Accounts (HSAs) or Flexible Spending Accounts (FSAs) can be used to pay membership fees. Check with your plan administrator for details.
You may cancel at any time with a 30-day written notice. We’ll help ensure your care transitions smoothly.
Occasionally, minimal fees apply for certain supplies or outside services (such as labs or pathology). You’ll always receive transparent pricing before any service is performed
