DPC DISCLAIMER

  • DPC is a membership-based model and is not health insurance.
  • Patients are encouraged to maintain insurance for emergency care, hospitalization, specialty visits, surgeries, or imaging that occurs outside the clinic.
  • Membership includes routine and chronic care services provided in-clinic, but does not cover outside lab fees, imaging, pathology, medications, or specialist care.
  • Some services may incur additional transparent fees, which will always be discussed in advance.
  • DPC membership does not replace the need for major medical coverage or catastrophic insurance.
  • Membership can be canceled with a 30-day written notice.
     

HRT DISCLAIMER

  • HRT is individualized and requires regular monitoring, including lab work and follow-up visits, to ensure safe and effective treatment.
  • Treatment outcomes vary by individual and cannot be guaranteed.
  • Certain medical conditions or medications may limit eligibility for HRT or require modified dosing.
